The mission of C & C Health Care Services is to enhance the well-being of the community by providing comprehensive, high-quality health care services including prevention and education with special emphasis on the medically under served and low-income populations. Our goal is to provide a “medical home” for the under served by significantly expanding access to primary care, behavioral health, addiction, disease management, prevention and educational services.
